Dental Appointments

We recognize that your time is important, and will do our best to ensure that your visits to our office are both pleasant and efficient.

If you are unable to keep your scheduled appointment, please call our office as soon as possible to reschedule the appointment. We ask that you arrange your schedule in such a way that you are able to make it for every appointment that you have scheduled. Any changes in an appointment with less than 24-hour notice are considered a “last minute cancellation.” If there is need for a last minute cancellation, you will be reminded about this policy. If there is a second cancellation within 24 hours of your scheduled appointment, you may be charged for that appointment.

Insurance

We are an in network specialty practice with most insurance PPO plans. We are out of network with HMOs and DMOs. However, you might still be entitled to benefits, so if you have dental insurance, please bring your plan information with you, and we will make sure that you receive the maximum benefits your dental policy or health flex fund provides.

As a courtesy, our office is happy to complete any necessary forms and submit to your insurance company for reimbursement and or pre-determination requests on your behalf. Please be aware that most dental insurance policies are limited, and will often only pay for a portion of any treatment. Dental insurance is intended to cover some, but not all, of the cost of your dental care. Most plans include co-insurance (co pay), deductibles, and other expenses which are the responsibility of the patient.

Financial

At Sunrise Periodontics, it is our goal to provide our patients with the finest and most comprehensive periodontal services available. Working with you to achieve your best oral care is our top priority, so we have outlined the following payment policies:

Unless other arrangements have been made, full payment is expected at the time of your procedure. We will always provide a detailed statement outlining your responsibilities prior to any procedure, so that you can arrive at an informed decision.

For your convenience, we accept cash, checks, money orders and the following credit cards:   MasterCard, Visa, Discover, and American Express.

For patients who need financial assistance, we are pleased to work with American Dental Association-endorsed payment programs such as Lending Tree and Care Credit, both of which offer affordable payment programs. Please speak with our knowledgeable staff if you have questions about your financial options.
